Medical Case Management

Sorrells Concierge serves as an advocate to individuals with chronic illness or disabilities and a liaison between individuals and their providers. Our staff is equipped with the knowledge to assist families and individuals with navigating through out the complexities of heath system.

 

Nursing Visits

Sorrells Concierge provides skilled nursing services to individuals who are in need of illness management to prevent hospital readmission or accidental injuries. Additional services include but are not limited to laundry services, feeding, personal hygiene care, dressing and grooming assistance, showers and 24 hours caregivers.

In home companion care, Alzheimer’s and adult sitting, meal preparation, light housekeeping, respite care and hospice companion care, grocery shopping, recreational activity supervision, companions for social events.
